We want to hear the views patients have about their GP services.

Each year NHS England carry out a survey of patient experiences at their GP practice.

Patients are selected at random.

If you are sent a questionnaire, we would really appreciate it if you could complete it and return it.

It is really useful to have feedback from patients on their experiences.
