Activity on Referral
Activity on Referral is a scheme by Trilogy which offers reduced price gym membership and is available to people over 16 years of age who are currently inactive.
Patients can be referred for a range of reasons including;
- overweight/obese
- issues with smoking
- depression
- anxiety
- mild bone and joint problems
- patients with Parkinson's
- MS
- diabetes
- asthma
- hypertension
- back pain

If you would like to apply for this scheme, please email the surgery to request a Trilogy AOR referral with your name and date of birth. If you have not provided us with your height and weight recently, please also include this in your email request so that we can provide the Trilogy team with your most up to date information.
